Please fill out the form below by September 2nd, 2024. You may dedicate a full memorial page for $180 or you may memorialize an individual name on the regular pages for $36 and any additional names for $18 each.

Each of the following sections costs $36 to participate in, but you can be a part of all three for $72. The book will include a memorial page dedicated to Holocaust victims, where you can have your name or your family's name included. Additionally, there is an option to honor the eleven Pittsburgh Kedoshim with your name. New this year, you can also include your name in the memorial page for all the victims of October 7th. We wish you and your loved ones a happy and healthy New Year.
